Chiropractor

Hassan Baqir D.C.

Dr. Hassan is passionate about helping his patients reach their optimal health through natural approaches such as chiropractic, exercise, and nutrition. He enjoys working with patients of all age groups and lifestyles.

Education: Palmer College of Chiropractic, University of Illinois - Chicago

He can work with and help a wide variety of patients, from high-level athletes to newborn babies. Dr. Hassan encourages a multi-disciplinary approach to healthcare and works closely with several primary care physicians, orthopedic doctors, and neurosurgeons in the surrounding community.

Dr. Hassan was born in Pakistan, raised in the Chicago suburbs. He is a die hard Bears fan and loves to watch them play. In his spare time, Dr. Hassan enjoys spending time with his family, playing cricket, and being an aquarist.

Whether you have a new injury or are growing frustrated with an old one, you can rest assured that Dr. Hassan will do all of the necessary tests and treatment to get you back to functioning at your highest level.

Alvin Ibalio

Alvin has worked as a nurse for over 20 years. His experience in the medical field has made him an asset to the Rejuvenate health team. Alvin has amazing interpersonal skills and interacts with everyone on many levels. He shows genuine interest and concern for our patients as well as the team.

marketing and community outreach

Anna Dziadkowiec

Anna is the marketing and community outreach liaison for Rejuvenate Health. She has over 25 years of experience in general surgery and brings a unique blend of medical knowledge and holistic care to chiropractic and functional medicine.

She is driven by compassion and community involvement inside and outside the clinic. Her values include continuous personal growth with the aim to help others feel their best. Her days are fueled by coffee, intention, and commitment to creating balance - whether it’s through wellness, movement or meaningful connections.

Outside the office, she enjoys gardening, grounding, and the outdoors as much as possible. She is also fluent in the Polish language.

Halyna Kurkovych

Halyna has over 8 years of experience as a massage therapist. She specializes in techniques such as lymphatic drainage, relaxing, sports, and hot stone massage. Her approach focuses on creating a personalized experience that promotes relaxation, healing, and overall well-being.

IV Infusion Expert & Functional Medicine Coach

Sophie Chudzynski

Sophie is an IV Infusion Expert with over 8 years of hands-on experience in IV therapy and patient care. She has performed thousands of infusions with a strong focus on safety, precision, and patient comfort.

Physical Therapy Assistant

Malak Halim

Malak is a physical therapist assistant who works closely with our patients throughout their rehab programs, making sure they’re using proper form and getting the most out of each session. She loves seeing our patients progress, gain strength, and improve over time. Malak brings a supportive and easygoing approach that helps make the rehab process both effective and encouraging. Outside of work, she enjoys playing volleyball and spending time with friends and family.

Sarah is the Office Manager at Rejuvenate Health, serving patients throughout their visit, whether at the front desk with their appointments or assisting in their exercises or answering all the tough insurance questions. She is passionate about wellness and is dedicated to helping others feel supported and cared for during their visit.
